A Critical Link to Patient Safety

Insufficient communication between hospital staff care adversely affects the quality and continuity of care provided and contributes to adverse events and increased health care costs. With the number of hand-offs involved in the day-to-day operations of patient care, the potential of error due to the communication process is considered a high-risk process. According to the Institute of Medicine (IOM) in two studies, at least 44,000 people and perhaps as many as 98,000 people die in hospitals each year as result of medical errors that could have been prevented. The IOM of 1999 called for a national effort to make health care safer. Standardizing the hand-off communication process is a necessity in the promotion of patient safety.

This webinar is designed as an educational program outlining the advantages of standardizing hand-off communication in an effort to improve patient safety in healthcare facilities.
